Getting to our clinic is easy and convenient, whether you’re driving, taking public transportation, or are eligible for free Medicaid-provided transportation.

We’re located at 28 Madison Street in Oak Park at the corner of Madison and Humphrey, with free parking available just north of the clinic.

  • Free parking is available in the lot immediately north of the Clinic, accessible from Humphrey Avenue beyond the alley. Village of Oak Park paid parking is available along Humphrey Avenue and Madison Street.

  • The Clinic is easily accessible by public transportation. We’re conveniently located four blocks from the CTA Green Line – Austin station, and five blocks from the CTA Blue Line, Austin station. Or the Austin 91 bus. Get directions

    For personalized trip planning and detailed schedules, you can contact the RTA Travel Information Center at (312) 836-7000 or visit transitchicago.com. The center operates from 6 AM to 7 PM, Monday through Saturday.

  • Did you know that you can arrange free transportation through your Medicaid Managed Care plan? Families who use the service share it is easy to call and schedule appointments. 

    • Call the number on the back of your membership card (if you don’t have your card, Google the phone #)

      • Call at least 72 hours in advance of routine appointments.

      • If your medical provider verifies your appointment is urgent, you can request a same day or next day ride.
